Pharmacist Hybrid General Practice and on site Pharmacy

Job summary

We are seeking an enthusiastic and clinically confident Pharmacist to join our team in a hybrid role, working across our General Practice and onsite (community) pharmacy. This is a great opportunity to deliver endtoend medicines optimisation supporting patients directly in primary care while ensuring safe, effective dispensing and clinical services in the pharmacy.

Youll be a GPhCregistered pharmacist who thrives in multidisciplinary teams, enjoys patientfacing work, and is keen to develop advanced clinical skills in line with NHS priorities (QOF, IIF/PCN outcomes, CVD prevention, respiratory, LTCs).

Main duties of the job

  • Conduct Structured Medication Reviews (SMRs) and postdischarge medicines reconciliation.
  • Manage repeat prescribing, eRD, formulary adherence, and costeffective prescribing.
  • Run clinical sessions (e.g., hypertension treatment escalation, asthma/COPD optimization, polypharmacy, deprescribing).
  • Support QOF, IIF/PCN objectives and quality improvement projects/audits.
  • Provide clinical advice to GPs, nurses, HCAs and care coordinators; contribute to MDT huddles/meetings.
  • Handle medication queries from patients and care homes; deliver patient education.
  • Implement prescribing safety measures (e.g., highrisk drugs monitoring, MHRA alerts).
  • Develop and update practice protocols/SOPs; contribute to clinical governance.
  • (If IP qualified) Provide independent prescribing clinics within scope of competence.
