We are delighted to offer a unique opportunity for an Advanced Nurse Practitioner to join our small, friendly team at Yorkleigh Surgery.
The Practice prides itself on creating a warm, welcoming environment for staff and patients. We provide quality clinical care to approximately 7900 patients and have worked to establish close connections within the local community, including nursing and residential homes, pharmacies, the District Nursing Team and other healthcare professionals.
We are also proud to be part of Central PCN, named 2023 PCN of the Year at the General Practice awards.
Hours to be worked on a rota basis, Monday - Friday, between the hours of 8:00am - 18:30pm.
Interviews are expected to take place week commencing 13th October 2025.
Main duties of the job
- To act as an autonomous practitioner within a busy GP surgery.
- To actively participate in the development and delivery of consistently high standard of patient care.
- Treatment Room Experience also required.
The post holder will work within a team of GPs, Nurse Practitioners and Practice Nurses as an autonomous practitioner and be accountable and responsible for the health and well-being of the patient group.
The post holder will have the ability to assess, diagnose and treat or determine the most appropriate pathway for the patient.
About us
Our clinical team consists of a GP Partner, three salaried GPs, two Practice Nurses, one HCA and one Phlebotomist as well as the extended team of Pharmacists, Social Prescribers and First Contact Practitioners.
They are supported by a committed team of care navigators, administrative support and managerial team.
As a smaller team we have to work collaboratively to ensure we are achieving the high standards that we attain to and are providing care to our patients to be proud of. To encourage an ethos of us working together as a team, we arrange bi-monthly socials, have small gifts at special times of the year and have a big, post Christmas celebration.

To deliver urgent care on the day (face to face consultations)
- To deliver expert advice to patients and relatives in order to promote health and protect their well-being.
- Receive patients with undifferentiated and undiagnosed problems.
- Exercise a high degree of professional autonomy and critical judgement to make differential diagnosis using decision making and problem solving skills.
- To provide extended skills in the management of a wide range of minor illness/injuries and ailments.
- Develop with patient and/or carer ongoing nursing care plan for health, emphasising prevention, utilising the wider health and social care services appropriately.
- Initiate and interpret tests and investigations as appropriate.
- Develop and utilise skills in nurse prescribing.
- Refer to a broad range of professionals across health and social care internally and externally as appropriate. All referrals must be discussed with a GP
- Act as a patient advocate.
Management of Chronic Disease (within area of speciality)
- Ensure that their practice is current with up to date practice and following current guidelines.
- Monitoring and development of individual management plan; agreeing these as appropriate with the patient and other health professionals.
- Recognising abnormalities and acting accordingly.
- Identifying the impact of treatment and implementing or recommending changes as appropriate.
- Check compliance and the adherence to appropriate treatments using a holistic patient-centred approach.
- To carry out diagnostic and therapeutic procedures.

- Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research.
