Job summary
Main responsibilities:
The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence based practice for patients presenting with long term conditions and the provision of preventative health care to the practice population. As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. The post holder will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures, and provide nurse leadership as required.
Main duties of the job
You will work alongside the nursing team to achieve CQC compliance with items such as infection control, cervical screening and immunizations, fridge cold chain, stock of supplies and medicines, PDGs, management of chronic diseases, etc. Some of these tasks can of course be delegated to others within the HCA and nursing team.
Excellent communication skills (written and oral).
Strong IT skills, use of EMIS and Docman, Competent in use of outlook and Microsoft office.
Clear, polite telephone manner
Effective time management (Planning & Organising)
Ability to follow clinical policy and procedure
Experience with audit and able to lead audit programmes
Experience with clinical risk management
Problem solver with the ability to process information accurately and effectively, interpreting data as required
Ability to work under pressure / in stressful situations
