A fantastic opportunity has arisen at Riverside GP Surgery for a full-time Practice Nurse. You will be joining a dynamic and enthusiastic team in a welcoming, supportive environment. Riverside is a busy GP practice with a diverse patient list of approximately 9400 supported by a full complement of care navigators, GP's and a diverse allied health care team. Main duties of the job
The post holder at Riverside GP Surgery is responsible for the delivery of high-quality general practice nursing to the entire practice population. The role focuses on providing evidence-based care for patients with long-term conditions, as well as delivering preventative health interventions across the practice.
Working alongside a highly experienced nurse, the post holder will develop and refine their clinical skills, applying critical thinking and autonomous decision-making in patient management. They will also have the opportunity to oversee and mentor student nurses, supporting their learning and development within the practice environment.
The nurse will work collaboratively with the wider general practice team to meet patient needs, ensure adherence to practice policies and procedures, and provide nurse leadership as required, contributing to the delivery of excellent patient care in a supportive and dynamic setting.

The Primary Care Nurse will provide comprehensive nursing care to the practice population, delivering evidence-based treatment for long-term conditions, preventative healthcare, and health promotion initiatives. The role includes supervising and supporting student nurses and working alongside experienced nursing staff to ensure high-quality patient care.
Key Responsibilities:
- Deliver holistic nursing care to patients with long-term conditions (e.g., diabetes, hypertension, COPD, asthma).
- Provide preventative care, including immunisations, screening, and lifestyle advice.
- Undertake clinical assessments, develop care plans, and make autonomous clinical decisions where appropriate.
- Work collaboratively with GPs, pharmacists, and allied health professionals to coordinate patient care.
- Supervise and mentor student nurses, supporting their learning and professional development.
- Maintain accurate patient records in line with practice protocols and governance requirements.Contribute to the development and implementation of practice policies and procedures.
