Job summary
The Practice Nurse will undertake a range of nursing assessments and provide appropriate care and treatment in conjunction with GPs, Nurse Practitioners, and other practice nurses or independently, in line with competence and need. Responsibilities include:
- Maintaining and improving standards of care, identifying patient population needs.
- Applying knowledge related to the Quality and Outcomes Framework (QOF).
- Delivering therapeutic interventions within a broader health promotion/public health strategy.
- Providing education and training on chronic clinical conditions.
- Offering specialist advice to patients and relatives regarding COPD, Asthma, and, where qualified, Diabetes.
- Engaging in further training for role development.
- Practicing within the NMC Code of Professional Conduct and maintaining accurate records.
Main duties of the job

The practice offers a variety of long term condition clinics and reviews. The nursing team are involved in the "check and test" component of the NCL long term conditions review initiative.
The practice nurse leads on infection control agendas.

About us
The Keats Group Practice, located in Hampstead, is
seeking an experienced Practice Nurse to lead our busy and friendly practice.
This fixed term contract will be to provide maternity cover for a period of 12 months and requires a commitment of 30 to 37.5
hours per week.
We are searching for someone who is passionate and motivated about General Practice. The ideal candidate will demonstrate exceptional
leadership skills and an ability to guide our team in delivering high-quality,
compassionate care to our patients. Our list size is around 13,500 patients.
Our practice team is made up of over 40 individuals, including 3 GP Partners, Salaried GPs, Practice Nurse, Healthcare Assistant, Physician Assistants, Pharmacists, Social Prescriber,
and a large reception and administrative team. We are an established training practice part of an Approved Learning Environment and committed to providing supervision and training to all of our clinical staff.
The practice nursing team are supported by one of the GP partners who provides weekly supervision. There are strong links with nursing colleagues within the local PCN and they meet on a quarterly basis to share learning. As part
of our dedication to continuous improvement, we maintain numerous external connections
and collaborate with PCN and ICB colleagues to further enhance our practice
services.
