Job summary

Come and join the Midway Surgery GP practice team. We are a really friendly and supportive team and this is a very important post which helps us provide the best medical care to our patients by providing excellent administration support by offering clinical tests and reviews as appropriate to patient needs.

Main duties of the job

You will identify patients needs using our efficient clinical IT systems such as vaccinations, health checks and reviews, blood tests and more, and will contact them to encourage engagement and offer appointments.

We would expect you to use your own initiative, be a keen and quick learner and be able to communicate confidently with our patients via telephone, text and letter.

GP Practice Care Co-ordinator

Accountable to: Practice Manager

Hours: 37 hours

Base: Midway Surgery

JOB SUMMARY

This is an important post which helps with the provision of the best medical care for our patients by providing excellent administration support. The post holder must be able to use their own initiative, be a keen and quick learner, be able to communicate well with our patients and be a positive and efficient member of a friendly team.

Utilising our efficient clinical IT systems, this post holder will ensure that patients needs are identified and that our patients can access the services appropriate to them such as vaccinations, health checks and reviews, blood tests and more. Excellent communication skills are needed to provide care for our patients and their families.

Experience in health or social care preferable.

Prior office experience is desirable to ensure efficient handling of administrative tasks and effective communication

Being proficient in the use of spreadsheets such as Microsoft Excel, using Word and having experience in computer systems would be an advantage.

Training and support will be provided.

DUTIES

Monitoring the clinical recall IT system and spreadsheet to offer clinical testing and invitations for appropriate clinical needs or reviews as per Practice protocols.

Respond to patient queries and contact patients to make appointments with relevant clinicians and encourage engagement where appropriate.

Take responsibility for certain patient care pathways such as Asthma and COPD, ensuring annual reviews are offered and appointments are made.

Work closely with the clinical team to provide computer searches and data to meet its service delivery requirements and changing care pathways.

Respond to data provided by national databases and update patients medical records accordingly.

Assist and participate in monthly inspections and audits.

Work closely with the Practice team in the coordination of work and daily administration needs as requested.

Attend relevant training meetings and training events, collaboratively with other Practices, IT colleagues and health and social care teams as required

Develop policies, procedures and patient information as appropriate to this role.

Provide training and support as appropriate to the Practice team.

Assist the Practice in providing a high-quality patient service, meet and demonstrate its performance markers and maximise efficiencies and resources.

Make recommendations relating to innovative ways of working, changing services, regulation updates, continuous learning and more.

Ensure that the areas of responsibility are safe and within legal and national regulations to include GDPR data protection regulations and Care Quality Commission compliance.

Some flexibility with hours will be needed particularly for meetings and training.

Other duties as deemed reasonable for this role.
