Job summary:
The practice nurse will
work as part of the primary healthcare team providing Personal Medical Services
to the patients of practice.
Job responsibilities:
A fully
qualified practice nurse will be competent in delivering the following
services. It is recognised that they might not yet have all
these skills and as such training will be given as appropriate however a diploma in diabetes management is an essential requirement.
- Assist in and
perform routine tasks related to patient care as directed by GPs
- Cytology
- Wound care /Leg
ulcer management /Doppler
- Removal of
Sutures/clips
- New Patient
Checks/Urinalysis
- Travel Clinic
- Childhood
Immunisations
- ECGs
- INRs
- Venepuncture
- Hypertension
Management
- Ear care/syringing
- Routine immunisations
- Smoke cessation
- Chaperoning and
assisting patients where appropriate who are being examined by another
clinician
- Assisting GPs
with minor surgery and IUCD/Implant fittings
- Requesting
pathology tests, for example urine culture, swabs
- Interest in management
of one or more of the following long-term conditions such as Diabetes,
Asthma, Coronary Heart Disease, Hypertension COPD
- Support the
Nurse Clinical Lead and Senior Nurse by developing clinical leadership in some
areas with agreement by the Nurse Clinical Lead
- Create and add
to care plans for patients with certain conditions such as leg ulcers,
diabetes, asthma, avoiding unplanned admissions, dementia etc.
- Documenting all
consultations and onward referrals appropriately within the clinical
system.
- Following
Safeguarding Adult and Child procedures as required including contributing
to case conferences or documentation if required.
- Following agreed
clinical protocols with referral to GPs as appropriate
- Ensure that
medical records are completed at time of consultation and are accurate and
that confidentiality is respected in line with surgery policy
Other tasks will
include:
- Maintaining and
cleaning equipment used by the nurses and GPs. Ensure cleaning rotas are completed and
signed for on a daily/weekly/monthly basis as per cleaning schedules.
- Maintaining GP
and Nurses rooms, stocking and rotating items as required
- Chaperoning and
assisting patients who are being examined by another clinician.
- Maintain general
tidiness and cleanliness of nurses and treatment rooms
- Maintain stock
of waiting room leaflets as instructed by partners and Practice Manager
- Ordering of
Health Promotion Literature
- Ordering of
stock for nursing requirements as agreed within the nursing team
- Ordering of
vaccinations to maintain stock levels
- Participation in
administrative systems in the Practice
- At all times
there will be a need to maintain accurate records
- A duty to advise
GP(s) of potential problems or errors within the range of assigned tasks
- Attend
and participate in any Practice meetings when required. These may be on days you do not normally
work, however you are expected to attend and will be paid overtime.
- On occasion
there may be a requirement to undertake home visits
- Any other
delegated duties appropriate to the post
