Job summary
The role of Practice Nurse involves the delivery of general practice nursing to the whole of the practice population. The focus is on both the delivery of evidence-based practice for patients presenting with long-term conditions such as Asthma, COPD and Diabetes etc, and the provision of preventative health care.
As an autonomous practice as a nurse you will be responsible for the care delivered, and demonstrate critical thinking and skills in clinical decision-making in the management of patients.
The post holder will work collaboratively within the general practice team to meet the needs of patients, support the delivery of policy and procedures and support the clinical team as required.
Main duties of the job
With regard to clinical practice you will assess, plan, develop and implement and evaluate treatment programmes that promote health and well being for patients, and those with long-term conditions.
You will provide core GMS Services such as Cytology, Vaccination and Immunisation Programmes including influenza etc.
You will proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate.
You will work with patients to support adherence to prescribed treatments and review medication for effectiveness appropriate to a patients needs and in accordance with national protocols.
Provide information and advice on prescribed or over the counter medication on medication regimes, side effects and interactions.
About us
We are a friendly and support practice comprising of 34 staff in total. This includes GPs, ANP, Nurses, HCA's, Secretaries and administrative staff.
We are based in the Dearne area of Barnsley and work across three sites, which are all modern and fit for purpose buildings. The main surgery is the Goldthorpe and we have branches at Thurnscoe and Great Houghton.
We come under the NHS Barnsley Clinical Commissioning Group and are a member practice of the Barnsley Healthcare Federation.
As a practice team we are committed to providing the best possible care/treatment for our patients. We provide both core and enhanced services to meet the needs of our patients.
