Job summary

We are looking to recruit an Advance Clinical Practitioner to join our friendly team at Leicester Terrace Health Care Centre.

We are seeking a qualified ACP to work with Adults and Children who is enthusiastic, approachable, compassionate and organised. You must be used to autonomous working and be self-motivated with a high standard of knowledge of working with a GP surgery.

This is a permanent position where we will offer excellent pay for the right candidate. Working hours for the role are negotiable.

A basic knowledge of computers is essential. Previous use of SystmOne would be an advantage however full training will be given.

Main duties of the job

The ACP will work as part of a multidisciplinary GP practice team to deliver safe, effective and autonomous clinical care. The post holder will assess, diagnose, treat, and manage patients presenting with acute, urgent, complex or undifferentiated problems across the age spectrum, including minor illness, minor injuries and paediatric presentations. They will also contribute to the home-visiting service, providing same-day care to housebound patients.

Job responsibilities

The ACP will demonstrate advanced clinical decision-making, critical reasoning, and the ability to work both independently and within the wider primary care team. The role incorporates all four pillars of advanced practice: Clinical Practice, Leadership & Management, Education, and Research.

Key Responsibilities

Clinical Responsibilities

  • Provide autonomous assessment, diagnosis, treatment and evaluation for patients presenting with minor illness, minor injury, urgent/unscheduled needs and undifferentiated symptoms.
  • Undertake paediatric assessments and manage acute conditions safely.
  • Conduct home visits as part of the practice visiting team, including frailty, long-term conditions, and urgent presentations.
  • Perform clinical examinations, interpret diagnostic tests (e.g., bloods, ECGs, imaging reports within competence) and make evidence-based decisions.
  • Manage and treat wound injuries, simple MSK injuries, sprains/strains and minor trauma.
  • Prescribe medications safely and appropriately in accordance with scope and legislation (independent prescribing desirable/required depending on applicant).
  • Make appropriate referrals to secondary care, community services or internal clinicians.
  • Provide patient education, health promotion and self-management advice.
  • Maintain accurate, timely and legally compliant clinical records.
  • Work within professional codes of conduct and scope of practice.

Leadership & Service Development

  • Support the day-to-day clinical workflow and contribute to practice operational efficiency.
  • Act as a clinical advocate for high-quality, evidence-based care.
  • Contribute to the development and implementation of clinical pathways and service improvements.
  • Participate in risk management, incident reviews and governance processes.

  • Registered with NMC / HCPC / GPhC
  • MSc in Advanced Clinical Practice (or equivalent Level 7 ACP qualification)
  • Evidence of advanced clinical assessment and diagnostic skills
  • Clinical portfolio demonstrating competence across ACP domains

Desirable

  • Independent Prescriber (V300)
  • Teaching/mentoring qualification
  • Minor injury/trauma training including MSK assessment

Experience

  • Significant post-registration experience in primary care, urgent care or similar environment
