Role: North Lambeth PCN Clinical Pharmacist
We are looking 1x enthusiastic, highly motivated Clinical Pharmacist to join our team and contribute to developing our services. Applicants for this specialist pharmacist role must be suitably qualified and will preferably have experience in an acute hospital setting or a GP Practice. The successful candidate will be required to work at any of the Practices within the North Lambeth Primary Care Network
Main duties of the job
This role will
also support the medicines management aspects of this initiative through
working within the general practice setting with the support of a Band 8
Clinical Pharmacist. This role will work as part of the general practice team
to provide specialist advice to patients, and undertake full patient cantered
medication reviews to proactively manage people with complex polypharmacy,
particularly the elderly and those with multiple conditions.
Candidates should be
a registered pharmacist with post registration experience, have well developed
communication skills and a patient focused approach to problem solving.

- Masters degree MPharm
- Registered Pharmacist MRPharmS
- Mandatory CPD to maintain fitness to practice
- Postgraduate Certificate in Clinical Pharmacy or Pharmacy Practice (or equivalent)
- Working towards a Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice (or equivalent)
Desirable
- Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice
- Member of the RPS faculty
- Independent Prescribing Qualification
Experience
Essential
- 2 years qualified with appropriate post registration experience in hospital or GP practice pharmacy or equivalent area of practice with assessed competencies
- Previous clinical pharmacy experience in relevant area
- Previous clinical pharmacy experience in Acute and/or GP Practice
- Experience of supervising others
- Experience of practice research or audit
- Experience of protocol/guideline
- Experience of delivering training
- Experience of directorate work
- Willingness to develop advanced practice using evidence mapped to the RPS Advanced Practice Framework
