An exciting opportunity has arisen for the role of Patient Flow Coordinator at NHS Practitioner Health. As Patient Flow Coordinator you will ensure that patients move smoothly through our service without delay. Main duties of the job
As a Patient Flow Coordinator, you will play a vital role in ensuring that patients receive timely and seamless care. Your responsibilities will include tracking and coordinating patients journeys across our multiple service offerings, ensuring follow-up appointments are scheduled, outcomes are recorded, and discharges are processed efficiently. Working closely with our Patient Services and Data teams, you will address any capacity issues, monitor patient flow, and support clinicians by identifying patients in need of follow-up. Through these efforts, you will help to minimise delays, improve patient experience, and facilitate optimal care pathways.
