Patient Care and Management:

    • Provide expert nursing care and management for patients with long-term conditions such as diabetes, asthma, COPD, and cardiovascular disease.
    • Conduct thorough health assessments and develop personalised care plans in collaboration with patients and their families.
    • Monitor and review patients' progress, adjusting care plans as necessary to ensure optimal management of their conditions.

Health Education and Promotion:

    • Educate patients and their families on managing chronic conditions, including medication management, lifestyle modifications, and self-care techniques.
    • Promote health and wellbeing through patient education sessions, workshops, and community outreach initiatives.

Collaboration and Teamwork:

    • Work closely with the multidisciplinary team, including GPs, healthcare assistants, and other allied health professionals, to deliver integrated and coordinated care.
    • Participate in team meetings and case discussions to ensure a holistic approach to patient care.

Quality Improvement and Professional Development:

    • Engage in quality improvement initiatives to enhance patient outcomes and service delivery.
    • Stay up-to-date with the latest clinical guidelines and best practices in managing long-term conditions.
    • Participate in continuous professional development opportunities to maintain and enhance clinical skills and knowledge.

Clinical Governance and Compliance:

    • Adhere to clinical governance standards and practice policies to ensure safe and effective patient care.

Qualifications and Experience:

  • Registered Nurse with current NMC registration.
