Job summary

We have an exciting opportunity for a Clinical Pharmacist to join our PCN. The successful candidate will be proactive and enthusiastic individual to join our established pharmacy team. The candidate will also work closely with the wider clinical team to help deliver safe and high-quality medicines management for our patients by having up to date clinical knowledge.

We are also seeking a commitment to addressing health inequalities and proactively reaching people from all communities. This can include managing long-term conditions (working directly with patients to assess and treat conditions) as well as promoting self care and so thecandidate will have excellent written and verbal communication skills.

Main duties of the job

The successful candidate will work as part of a multi-disiciplinary team to develop and run processes for repeat prescription re-authorization, management of medicines on transfer of care and systems for safer prescribing. You will also undertake structured medication reviews for patient in care homes and community.

About us

Wolverhampton North Network is a successful PCN, consisting of 7 practices and over 50,000 patients. The successful candidate may need to be able to travel to all sites.

The applicant can expect to work within a supportive environment where you will receive regular clinical supervision, and will be actively encouraged to undertake further training and study in order for you to continue to develop professionally, in accordance with your agreed personal development plans. This includes support of a General Practitioner clinical supervisor and Senior Clinical Pharmacist.

Job description

Job responsibilities

  • Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
  • Be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.
  • Provide specialist expertise in the use of medicines whilst helping toaddress both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities.
  • Provide leadership on person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.
  • Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote selfcare.
  • Have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload.
  • Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.
  • Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning disabilities), liaison with community pharmacists and anticoagulation.
  • Be part of a professional clinical network and have access to appropriate clinical supervision.

  • Ability to critically analyse complex information and evidence and communicate the findings clearly in simple terms
  • Excellent clinical knowledge and ability to apply to practice
  • Demonstrates understanding of legal, governance and ethical frameworks within which pharmacists and primary/secondary care organisation's work
  • Proven verbal and non verbal communication skills

Desirable

  • Experience of working in a GP practice / Primary Care Setting
  • Experience of undertaking evidence reviews
  • Experience of clinical audit
  • Ability to identify risks, anticipate issues and create solutions and to resolve problems in relation to project or service
