Job summary

Wolverhampton North Network have an exciting opportunity for a Health and Wellbeing Coach to join our already established wellbeing team and work alongside the wider multi-disciplinary team (MDT).

We are looking to recruit an enthusiastic and self-motivated Health and Wellbeing Coach to support our patients to take pro-active steps to develop the knowledge, skills and confidence to manage their physical and mental health and achieve lifestyle goals.

As a Health and Wellbeing Coach you will help patients develop the knowledge, skills, and confidence to manage their own health and wellbeing. You will use health coaching, wellbeing conversations and behaviour change skills to empower people to make positive lifestyle changes. The role will take a holistic approach that helps increases peoples ability to access community support and self-management education.

About us

Wolverhampton North Network is a successful PCN, consisting of 7 practices and over 55,000 patients. The successful candidate may need to be able to travel to all sites and have effective time management processes. The applicant can expect to work within a supportive environment on a variety of exciting projects.

Job responsibilities

As a Health and Wellbeing Coach you will help patients develop the knowledge, skills, and confidence to manage their own health and wellbeing. You will use health coaching, wellbeing conversations and behaviour change skills to empower people to make positive lifestyle changes. The role will take a holistic approach that helps increases peoples ability to access community support and self-management education.

The Health and Wellbeing Coach will manage and prioritise a caseload, according to the needs, priorities and support required by individuals in the caseload. They will identify when a persons needs are beyond the scope of the health and wellbeing coach role and refer them back to other health professionals or organisations.

Key responsibilities

  • Manage and prioritise a caseload, in accordance with the health and wellbeing needs of their population through taking an approach that is non-judgemental, based on strong communication and negotiation skills, while considering the whole person when addressing existing issues.
  • Where required and as appropriate, the Health and Wellbeing Coach will refer people back to other health professionals within the PCN.
  • Utilise existing IT and MDT channels to screen patients, with an aim to identify those that would benefit most from health coaching.
  • Provide personalised support to individuals, their families, and carers to support them to be active participants in their own healthcare empowering them to manage their own health and wellbeing and live independently through:

(i) coaching and motivating patients through multiple sessions to identify their needs, set goals, and supporting patients to achieve their personalised health and care plan objectives.

(ii) providing interventions such as self-management education and peer support.

(iii) supporting patients to establish and attain goals that are important to the patient.

(iv) supporting personal choice and positive risk taking while ensuring that patients understand the accountability of their own actions and decisions, thus encouraging the proactive prevention of further illnesses.

(v) working in partnership with the social prescribing service to connect patients to community-based activities which support them to take increased control of their health and wellbeing.

(vi) increasing patient motivation to self-manage and adopt healthy behaviours.

  • Work in partnership with patients to support them to develop their level of knowledge, skills and confidence enabling them to engage with their health and well-being and subsequently supporting them in shared decision-making conversations.
  • Utilise health coaching skills to support to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.
  • Explore and support patient access to a personal health budget, where appropriate, for their care and support.
  • Develop collaborative relationships and work in partnership with health, social care, and community and voluntary sector providers and multidisciplinary teams to holistically support patients wider health and wellbeing, public health, and contributing to the reduction of health inequalities.
  • Provide education and specialist expertise to PCN staff, supporting them to improve their skills and understanding of personalised care behavioural approaches and ensuring consistency in the follow up of peoples goals with MDT input.
  • Raise awareness within the PCN of shared decision-making and decision support tools.

Experience

Essential

  • Ability to demonstrate management of own caseload and prioritisation of case work.
  • Able to interpret, translate and deliver key messages to patient groups
  • Good practical and conceptual knowledge of healthcare improvement methods and community level services for referral purposes
  • Experience of successfully establishing working relationships within teams across multiple locations
  • Ability to collect and record information and data, for record-keeping, monitoring and evaluation

Desirable

  • Experience or training in person-centred care planning
  • An understanding or previous experience of working within a Primary Care organisation or a comparable non-profit agency
  • Previous experience or providing an advisory service assisting with complex patient situations which require multi agency working

  • Hold a recognised healthcare or coaching qualification (or equivalent relevant experience in motivational interviewing)
  • Coaching/counselling experience
  • Good standard of education with excellent literacy and numeracy skills

  • Workforce Development Framework including training, or
