Job summary
Wolverhampton North Network PCN are looking for an experienced Diabetes Specialist Nurse to become key member of the core PCN team, working with practice teams and other professionals across the 7 different sites that are part of the Primary Care Network. Our PCN hosts a wide range of clinicians, including our Pharmacists, Physicians Associate, Care Coordinators, Mental Health Practitioner, Social Prescribers, First Contact Physiotherapist and many more.
We will provide continual training, support and supervision by the dedicated Clinical Team.
Main duties of the job
As part of our Health Inequalities work, the post-holder will provide excellent diabetes patient care to improve the health and wellbeing of patients with diabetes. This will include patients in our care homes.They will undertake care planning for patients with a diagnosis of diabetes, and promote self-care management.
The role will be an integral part of the general practice team working with our practices and across the PCN.

Wolverhampton North Network is a successful PCN, consisting of 7 practices and over 50,000 patients. The successful candidate may need to be able to travel to all sites and have effective time management processes.

- To promote self-care management to patients to prevent worsening of their condition and improve health outcomes.
- To ensure full understanding of QOF for diabetes chronic disease management and to liaise with the team to ensure targets are achieved.
- To actively collect and maintain data that will demonstrate patient improvement in key areas.
- To ensure delivery of best practice in clinical practice, caseload management, education, research, and audit, to achieve corporate Primary Care Network and local population objectives.
- To act as a specialist resource and provide specialist advice in relation to diabetes, to patients their family/carers and health professionals.
