Job summary
We have a new opportunity for a dynamic, forward-thinking, Advanced
Care Practitioner/Paramedic to join our Practice-based multi-disciplinary team.
The ideal candidate for this position possesses excellent
patient service skills, be comfortable working within a team, able to work well
under pressure and make quick decisions about patient healthcare needs.
The role includes triaging patients who wish to be seen by
a healthcare professional on the same day, diagnosing and treating patients
presenting with minor illness and managing patients in respect of their continuing medical needs on
the phone, at the practice or in the patients home.
Main duties of the job

Assess
patients by telephone triage, manage patients healthcare needs or refer to
colleagues where appropriate
As a
responsible autonomous practitioner, use local care pathways to see and treat
patients at the practice or in their own home
Make
reasoned decisions regarding initiating, modifying or ceasing treatment or
procedures and ensuring accurate documentation of decisions.
Being
accountable and responsible for clinical decisions made during the course of
duty, seeking senior clinical advice where appropriate.
Utilise
appropriate and relevant information to assist patients, their advocates or
carers to make informed health choices in line with practice, local and
national guidance, in accordance with health promotion and prevention agenda
Complete
all necessary reports and notifications as required by the Practice
Participate
in clinical audit and effectiveness process, attending and contributing to
clinical team meetings
About us
Bow Medical Practice serves a largely rural
population of 5,800 patients and offers dispensing services to our patient
community. We have a GP Partner and a non-clinical Managing Partner. We work
from a Partner owned modern, purpose-built building which we moved into in 2012
and beautiful views from the consulting rooms! We also have a branch
surgery four miles away in the town of North Tawton.
Our clinical system is EMIS Web; we offer 15 minute
appointments, a daily clinical huddle/coffee mid-morning and work hard to share
the workload between the whole team to maintain a good work-life balance.
We were rated Outstanding by CQC in 2016 and 2020.
We have maintained high QOF outcomes, there is no OOH commitment but
opportunity to be involved in PCN delivery if desired.

of Practice.
Working alongside colleagues to ensure the highest standards of care
for all patients, dealing with a wide range of presenting health needs in a
general practice setting
The
post-holder will provide clinical care to patients of the Practice by way of
(inter alia) surgeries, clinics, triage clinic, home visits and relevant
administrative work together with such other duties reasonably delegated to you by the Partners. Providing treatment
to patients in participation with general practitioners or independently to
agreed protocols.
Working
in accordance with the HCPC Code of Conduct, the post-holder will provide
assessment, treatment, screening, health education services and advice to
patients on general health care and minor ailments with referral to general
practitioners as necessary.
The post-holder will have the required level of
training and competence and will work within patient group directions where
these are available. Prescribing qualifications will be used appropriately.
The
post-holder will also contribute to practice meetings, planning and decision
making as part of a practice team (as appropriate) providing leadership to the
nursing team.
