Main duties of the job

The role of the pharmacist here at the surgery will resolve day-to-day medicine issues and consult with and treat patients directly. This includes providing help to manage long-term conditions, advising those taking multiple medicines (polypharmacy) and delivering clinical advice about treatments. They will also assist with communication across a patients care pathway, manage medicines shortages by suggesting suitable alternatives where appropriate

Job responsibilities

The post holder is a clinical pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice.

They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients.

The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. This includes being a conduit of patient medicines information into and out of hospitals. They will work to optimise medication issues to improve patient care and safety, and support clinical staff in the management of patients.

The supervision and mentorship network will be provided to support the clinical pharmacist.

  • Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role

Desirable

  • A Previous General practice position is desirable, however, those with no experience are encourage to apply as full training and induction will be offered

  • Independent prescriber certificate
