Job summary
We are pleased to advertise the role of Digital and Transformation Lead within Health Village Dearne Valley PCN - a Primary Care Network collaboration of 3 practices based within the Rotherham area serving a patient population of 48,000.
The post holder is to support all aspects of the development
and function of the Primary Care Network (PCN). They will work alongside the
Clinical Director, PCN lead and PCN Board members to build on current primary
care provision, implement improvement and transformation aligned to local and
national priorities and be committed to the shared purpose and vision of the
network.
Main duties of the job
The Digital and Transformation Lead will help to develop,
guide and oversee the strategic aims and objectives of the Network, ensuring
that Practices and stakeholders have the relevant information and options to
make informed decisions. The post holder is able to implement agreed plans and
projects into business operations, with the ability to plan, monitor and
evaluate practical improvements that will make a positive difference to
patients and Practices within the PCN.
Working closely with a wide range of partners and
stakeholders, both within the PCN and externally, the post holder will actively
seek out opportunities and projects in accordance with contractual requirements
and aligned with the PCNs strategy that will enhance patient care, address
health inequalities, and support the Practices with the PCN.
About us
Health Village Dearne Valley PCN is a collaboration of 3 practices based across Rotherham. We are an effective and efficient PCN with a successful track record of providing safe, effective and holisitic patient care.
You will work closely with the various Practice Managers across the 3 constituent Practices as well as alongside our Clinical Director. You will also work with, and oversee, a varied team of other PCN staff including Paramedics, Pharmacists and Pharmacy Technicians, Social Prescriber, Nurse Associate and Care Coordinators.

- Ability to provide inspiration, guidance and leadership to a range of stakeholders.
- Experience of managing complex funding streams and high-level contracts and the ability to interpret financial and contractual information for a range of audience.
- Experience of using programme management, methodologies and improvement techniques to drive change forward.
- Ability to utilise entrepreneurial skills to actively seek out opportunities that will positively impact the Networks Practices and patients.
- Ability to effectively resource manage Network employees and understand the implications within the wider Network.
- Ability to effectively negotiate and influence decision making using sound logic and researched evidence.
- Ability to work autonomously and manage own workload whilst managing a range of conflicting deadlines and priorities.
- Ability to work effectively and build and maintain good working relationships with a range of internal and external clinicians, managers, stakeholders and partners.
