Job summary

Mosborough Health Centre is seeking a motivated and experienced Pharmacist to join its multi-disciplinary team. The successful candidate will play a key role in medicines optimisation, clinical medication reviews, and the safe and effective use of medicines across the practice population.

Working both independently and collaboratively, the post holder will provide clinical and administrative support to ensure high-quality patient care, improved prescribing safety, and seamless transitions between primary, community, and secondary care.

Main duties of the job

Conduct structured clinical medication reviews for patients with complex polypharmacy, long-term conditions, or in care homes.

Undertake face-to-face and telephone consultations, supporting patients to use medicines safely and effectively.

For independent prescribers, prescribe in line with clinical guidelines and legal requirements.

Manage long term conditions and self-limiting conditions within competence, referring as needed.

Support repeat prescribing, acute requests, and medicines reconciliation following discharge or transfer of care.

Review prescribing of high-risk medicines to reduce unplanned hospital admissions.

Medicines Optimisation & Safety

Implement local and national medicines safety alerts (e.g., MHRA guidance).

Contribute to the Quality and Outcomes Framework (QOF) and enhanced services.

Audit prescribing trends and assist in maintaining compliance with NICE and local formulary guidance.

Provide expert clinical advice on medicine use and contribute to public health initiatives.

Education & Collaboration

Provide training and education for GPs, nurses, and the wider primary care team.

Liaise with community and hospital pharmacy teams to ensure continuity of care.

Participate in quality improvement and governance activities.

Engage with Patient Participation Groups (PPGs) and contribute to service development.

Qualifications

Essential

  • MPharm degree (or equivalent).
  • Registered with the General Pharmaceutical Council (GPhC).
  • Awareness of acute and long-term conditions commonly seen in general practice.
  • Excellent communication, teamworking, and problem-solving skills.
  • Ability to work autonomously, prioritise workload, and make sound clinical judgments.
  • Working knowledge of national guidelines and pharmaceutical standards.
  • Advanced IT skills and experience using clinical systems.

Desirable

  • Postgraduate Clinical Diploma or equivalent qualification.
  • Independent prescriber qualification.
  • Experience in general practice, hospital, or community pharmacy (minimum 1 year).
  • Familiarity with triage or unscheduled care services.
  • Membership of a professional body (e.g. Royal Pharmaceutical Society).
  • Completion of Primary Care Pharmacy Education Pathway (PCPEP) or MSc Advanced Practice Pathway.
  • Interest in polypharmacy management and improving patient outcomes.
