Sea City Partnership has an exciting opportunity to join our advanced practice team, providing health care to an undifferentiated caseload of patient presenting with acute and chronic health conditions, within our practice sites or in the patients own home.

Main duties of the job

  • To ensure that the needs of the patients are placed at the centre of care delivery
  • To deliver a high standard of patient care as an advanced practitioner in general practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base
  • To manage a clinical caseload of patients presenting with undifferentiated health needs, responding effectively to patient need and ensuring patient choice and ease of access to services.
  • The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers, and colleagues, ensuring clinical safety and excellence.
  • Prescribe safe, effective and appropriate medication as defined by current legislative framework
  • To undertake and promote confident, safe, and autonomous practice. To engage with continuous professional development and to work with the advanced practitioner and wider practice team to develop competencies to expand the role of advanced practitioners as a core part of the Sea City clinical team.
  • To remain clinically competent, registered with appropriate governing body as relevant to profession and ensure continuous professional development opportunities are undertaken to keep up-to-date and then disseminate clinical best practice guidance

Clinical Role

Function as a key member of the clinical team at Sea City Partnership, delivering primary care services to patients.

To deliver clinical assessments and primary care services for patients of Sea City Partnership, both remotely (over telephone) and face-to-face, including in the surgery and in the patients own home.

Provide scheduled (booked) care and on-day assessment for patients presenting to the practice with more urgent needs.

To undertake assessments as required in patients own home and other community settings as appropriate.

Make professionally autonomous decisions for which the post holder is accountable.

Manage an undifferentiated caseload of patients presenting with acute and chronic, complex health needs.

Contribute to the management of care for patients within a list based primary care model, contributing to an MDT/Clinical firm to promote continuity of care.

Manage their own clinical workload in general practice responding effectively to patient need and ensuring patient choice and ease of access to services.

Provide assessments and care for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.

Maintain accurate and contemporaneous records, utilising computer systems where appropriate and consider the Caldicott Principles in relation to all data handling.

Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.

Provide safe, evidence-based, cost-effective, individualised patient care.

Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care.

Prescribe safe, effective, and appropriate medication as defined by current legislative framework and local prescribing standards.

Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills.

Refer patients directly to other services/agencies as appropriate including but not limited to services of the Primary Care Network, acute hospital services, community services, mental health services and local social services.

Support the other clinicians within the practice in developing and delivering specialist primary care services as required.

Work within the practice guidelines, policies, and protocols.

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery.

Provide ongoing care to patients with more complex needs, including liaising with appropriate multi-disciplinary team to create appropriate care plans and review them as required.

Work within the latest Code of Professional Conduct relevant to profession.

To undertake continuous professional development and to work with the Advanced Practitioner team and wider team to maximise the safe scope of advanced practice within the St Marys based clinical team.

Keep up to date with pertinent health-related policy and work with the practice team to consider the impact and strategies for implementation.

Work collaboratively with colleagues within and external to the practice.

Pro-actively promote the role of the Advanced Practitioner within the Practice, and externally to key stakeholders and agencies.
