Job summary
We are excited to be offering this opportunity to join our Practice Nurse Team. The postholder will join our dedicated Practice Nurse team to offer patients a range of nursing services, including but not limited to vaccinations and immunisations, wound care and treatment room services and long term condition health-checks.
The ideal candidate will be a registered nurse and will ideally have some experience of nursing in general practice. We are also particularly interested in applications from nurses with experience and training in respiratory and or diabetes care.
The postholder will be required to work across all of our sites with a likely emphasis on our Mulberry House Surgery site in the first instance.
Main duties of the job
- Delivering high quality, compassionate, evidence based and person centred care to patients
- To deliver opportunistic health promotion where appropriate
- To provide long term condition reviews for relevant patients
- To administer medication for patients in accordance with Patient Group Directives
- To request pathology services and process pathology results as required.
- To ensure that records of consultations and other interactions with patients are maintained correctly
- To deliver women's health services (e.g. cervical smears)
- To review Med3 requests and take action as required
- To review reports from GP out-of-hours services
- To support other members of the practice clinical team with nursing services including supporting clinical emergencies
- To recognise, assess and refer/signpost patients to other members of the practice clinical team and/or external partner agencies as required
- To support the practice in offering student nurse placement mentoring, coaching and teaching
- To liaise with external services/agencies where necessary to ensure the patient is supported appropriately
- To safeguard children and vulnerable adults, ensuring that concerns are escalated appropriately in accordance with local and national policies.
- To support the Infection Prevention Control (IPC) lead and the implementation of the IPC Policy taking part in audits, training and support
- To participate in supervision, peer support and professional development arrangements
