Job summary

We have a new and exciting opportunity for an experienced clinician to join our established team of Nurse Practitioners and Paramedics. The post holder will work across all sites. The team provide acute minor illness care to patients and Home Visiting Service, aiming to prevent A&E visits and hospital admissions.

We are supportive and forward-thinking practice with an excellent clinical team. We are also a training practice for doctors and are supportive of personal development & ongoing training for all staff. We aim to provide our own succession planning.

Main duties of the job

Our Practitioners are supported to work independently, autonomously delivering care and treatment to patients within the clinical setting, having gained a wide range of knowledge and clinical skills in Primary Care. These skills will enable the post holder to provide the highest standard of care.

The successful candidate will undertake face to face clinical assessment and treatment of patients for minor illness, including undertaking telephone consultations and triage.

Applications are welcomed from experienced clinicians who have organisational skills and strong time management skills to ensure patients are seen in a timely way and that clinic run effectively, without compromising patient care or safety.

OPERATIONS MANAGER (Administratively)

Job Summary:

Alongside the GP, provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. To work across sites.

Provide assessment, treatment and diagnosis at point of first contact, by attending to patients in a variety of clinical or non-clinical settings according to patients needs.

Conduct telephone triage alongside the team.

Adhere to existing clinical protocols for Rapid Access and provide input to the wider Rapid Access team.

Make professionally autonomous decisions for which you will be accountable.

Instigate necessary invasive/non-invasive diagnostic tests/investigations and interpret findings/reports.

To be focal point/lead for emergency drugs and equipment within the practice ensuring most up to date algorithms, protocols, training needs and drugs being used.

Ensure that there is a robust system in place for maintaining clinical governance.

Work with multi-disciplinary teams within practices, healthcare community, to promote integrated and seamless pathways of care.

Refer patients to an alternative care setting or treat & discharge as appropriate.

Contribute to the locality practices achieving its quality and quantitive targets to sustain the high standards of patient care and service delivery.

Participate in the assessment of community health needs and help to develop patient and family-centred strategies to address them.

Help to develop and set up new patient services and participate in initiatives to improve existing patient services.

To use Surgery, Local and National guidelines and professional knowledge and expertise to inform an assessment of the clients health and presenting problem.

On the basis of assessment to treat or refer the client to the appropriate practitioner or service. Responding quickly to emergency or urgent situations.

To maintain accurate and timely record of all clinical consultations and ensure that the NMC/COP/HCPC Professional code of conduct is adhered to.

To maintain and develop professional practice, knowledge, skills and expertise to meet the needs of the practice.

To establish effective networks with other health professionals and outside agencies.

The post holder will be expected to work with a high degree of independence and manage his/her own workload according to priorities.

The post holder will participate in annual Performance and Development review (PDR).

Following agreed clinical protocols with referral to senior nurses or GPs as appropriate.

Able to document Consultations according to NMC/COP/HCPC guidelines.

Ability to obtain and document informed consent (either verbal or written).

Ability to recognise safeguarding issues of both child and vulnerable adults.

Able to recognise and manage anaphylaxis according to current UK guidelines.

Able to perform cardio-pulmonary resuscitation according to current UK guidelines.

Ensure infection control guidelines are maintained.

Chaperoning and assisting patients where appropriate who are being examined by another clinician.

Perform venepuncture according to local guidelines.

All prescribers at SMG must take individual responsibility for their prescribing decisions and should recognise that there are certain areas of practice where remote prescribing is unlikely to be suitable, for example when prescribing medicines likely to be subject to misuse or abuse.

A Non-Medical Independent Prescriber can prescribe any medicine for any condition within their clinical competence.

Other tasks will include:

Maintaining and cleaning equipment used.
