We are an expanding GP surgery in Horsham looking to recruit a caring and enthusiastic Senior Health Care Assistant to join our exceptionally friendly and hard-working nurse team due to the previous post-holder relocating. The post holder will provide care and support to our patients by undertaking a range of delegated clinical tasks, as well as assisting with the administration of patient cohorts, for example the booking of patient appointments for long-term health conditions.

The hours on offer are negotiable for the right candidate but as an example:

  • Mondays DAY OFF
  • Tuesdays - 07:00 to 17:30 - 1 hour lunch
  • Wednesdays - 07:00 to 17:30 - 1 hour lunch
  • Thursdays 07:00 to 17:30 - 1 hour lunch
  • Fridays 07:00 to 17:00 - 1 hour lunch

Courtyard Surgery currently has a population of 15,900 patients. We have a highly skilled and friendly nursing team that work closely with the other clinicians in the practice. Main duties of the job

The successful candidate will have excellent organisational and time management skills and be passionate about delivering holistic care that has the patient at its centre. The post holder will be supported to develop skills and abilities, both to enhance their career and to provide an appropriate level of care to patients. Applicants will have an NVQ Level 2 or 3 in Health and Social Care (or equivalent) and be able to demonstrate previous experience as a Health Care Assistant, either in primary or secondary care.

Salary equivalent to a Band 4 under Agenda for Change due to the seniority and experience required.

Job description

Job responsibilities

a. Support the practice nurse with health promotion programmes

b. Support the practice nurse with the management of Chronic Disease clinics

c. Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, calculating & recording average readings from SystmConnect & AccuRx, and making appointments as needed

d. Review patient triage submissions and making nursing / HCA appointments as needed, review incoming tasks to the nurses inbox and book where appropriate, for example SystmConnect submissions

e. Dip urine samples, record and share results as appropriate, and send to lab

f. Facilitate routine and 24-hour BP monitoring, advising patients accordingly

g. Undertake Wound Care, dressings and other clinical tasks as required

h. Undertake Suture/Clips removal

i. Carry out BMI checks as directed

j. Undertake annual Patient Health Checks

k. Act as a chaperone as required

l. Undertake venepuncture

m. Administer flu vaccinations

n. Carry out ECGs as requested

o. Carry out Shingles, Pneumococcal and B12 injections

p. Carry out NHS Health Checks

q. Carry out learning disability health checks

r. Diabetes screening to include foot checks

s. Provide support during minor operations as required

t. Ensure specimens are recorded and ready for onward transportation

u. Ensure all clinical rooms are adequately stocked and prepared for each session

v. Ensure all fridges are adequately stocked and prepared for each session

w. Attend fortnightly/monthly nurse team, clinical team and practice team meetings

x. Ensure fridges are cleaned routinely in accordance with extant guidance

y. Ensure fridge temperatures are logged daily, AM/PM

z. Ensure clinical waste is removed from clinical areas daily and sharps bins replaced in accordance with the practice IPC policy

aa. Deliver opportunistic health promotion where appropriate
