We have an exciting opportunity for an enthusiastic, caring, and highly motivated Practice Nurse to join our friendly and supportive team. This is an excellent opportunity for a committed healthcare professional who is passionate about delivering high-quality, patient-centred care within a thriving general practice surgery.

You will play a key role in providing and maintaining a high standard of nursing care to our patient population, working closely with GPs, senior nurses, and the wider multidisciplinary team. Courtyard Surgery currently has a population of 15,900 patients. We have a highly skilled and friendly nursing team that work closely with the other clinicians in the practice. We aim to provide personalised, proactive, high quality clinical and patient care. Our ethos is to treat everyone with compassion and respect, in a warm and welcoming environment.

Our patient demographic is mixed but weighted to working age patients leading to an interesting chronic disease workload and screening activity. We have a highly organised and skilled administration team who effectively signpost patients to other services and have an embedded workflow management system to reduce the administrative burden.

Main duties of the job

About you:

Essential

  • Registered Nurse with current NMC registration
  • Previous experience working in General Practice

Desirable

  • At least one years experience working within a nursing team
  • Cervical cytology (smear taker) qualification
  • Experience in immunisations and vaccinations
  • Experience in injections specific to individualised patients using PSDs
  • Wound care experience
  • Long-term condition management including diabetes, asthma and COPD
  • Experience in microsuction

Job description

Job responsibilities

a. To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the Nurse Manager and Practice Manager

b. To support the production of the IPC annual statement in conjunction with the Nurse Manager and Practice Manager

c. To support the development, implementation, and embedding of effective care management programme for the frail and housebound patients registered at the practice

d. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

e. To support the development, implementation, and embedding of health promotion and wellbeing programmes

f. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

g. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

h. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

i. As required, to diagnose and manage acute and chronic respiratory conditions

j. Provide wound care (Ulcer/Doppler etc.) to patients

k. Carry out physical examination and tests, such as cervical smears and pregnancy tests

l. Request pathology services as necessary

m. Process pathology results as required

n. To prescribe medication for patients in accordance with PGDs and own scope of practice

o. To review patient medications to enhance compliance

p. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

q. To provide chronic respiratory disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
