Job summary
We are an expanding GP surgery in
Horsham looking to recruit a Care Coordinator to join our exceptionally
friendly and hard working practice team.
As a Care Coordinator you will work closely with
GPs, clinical staff and the practice team to manage a caseload of patients,
acting as a central point of contact to ensure appropriate support is made
available to them and their carers; supporting them to understand and manage
their conditions and ensuring their changing needs are addressed.
Main duties of the job
In addition to practice team you will
also work with the wider multi-disciplinary team within the locality. You will
be required to have an active role in appropriate meetings as well as
organisation of appropriate updates to the clinical team regarding the care of
patients under your administration. You will need to possess excellent
communication skills, be comfortable dealing with patients and their relatives
and/or carers, and be able to prioritise your work effectively. You will be
joining and working closely with an innovative and supportive team of
healthcare professionals who are committed to providing the best possible care
for our patients.
Hours of work are negotiable but will need
to be within the core opening hours of the practice, Monday to Friday, 08:00 to
18:00.

Working closely with the practice and the multidisciplinary team (MDT), the role is pivotal in ensuring all patients receive the best possible care and service. The role will support the clinical staff in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. The role will have specific emphasis on supporting the Cancer Lead and the Cancer work stream within the practices. The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patients journey through primary care.

Workers and Health and Wellbeing Coach(es)
- To support patients to utilise decision aids, help create single
personalised care and support plans, in line with best practice
- To support the Clinical Director in the delivery of the DES

- To help people to manage their needs through answering queries, making, and managing appointments
- To provide coordination and navigation with the aid of digital tools for people and their carers across health and care services
- To support the coordination and delivery of MDTs within the PCN
- To work specifically with the Cancer Lead to deliver a supportive and
robust care work stream

evaluation and research to promote quality improvement activity
- Collaborates with other members of the MDT, patients and their carers when managing and coordinating care
