We are looking for an experienced and enthusiastic Practice Nurse to join our warm, closeknit team in a friendly rural village surgery. Main duties of the job

The successful candidate will provide a wide range of nursing services within the practice working both independently and as part of our multidisciplinary team.

Key responsibilities include:

Desirably chronic disease management e.g asthma COPD diabetes hypertension

Cervical cytology

Childhood and adult immunisations

Wound care and treatment room duties

Health promotion and lifestyle advice

Supporting patients with long-term condition self-management

Participation in QOF and other clinical targets

Contribution to clinical governance audits and service development

You will be expected to deliver evidence based care, maintain accurate clinical records and work collaboratively with GPs pharmacists HCAs and administrative staff.

Job responsibilities

To deliver highquality, evidencebased nursing care to patients within the GP practice, working both independently and as part of the multidisciplinary team. The post holder will provide a range of clinical services, support longterm condition management, and contribute to the ongoing development of nursing services within the practice.

Provide treatment room services including wound care, dressings, ear care, ECGs, blood pressure monitoring, and health checks.

Deliver cervical cytology in line with national screening standards.

Administer childhood and adult immunisations, ensuring safe storage, handling, and documentation of vaccines.

Provide health promotion, lifestyle advice, and patient education.

Support patients with longterm conditions, promoting selfmanagement and adherence to care plans.

Desirably undertake chronic disease management reviews (e.g., asthma, COPD, diabetes, hypertension).

Perform clinical observations, assessments, and escalate concerns appropriately.

Maintain accurate, timely, and comprehensive clinical records.

Participate in QOF, enhanced services, and other clinical targets.

Contribute to infection prevention and control processes within the practice.

Work collaboratively with GPs, pharmacists, HCAs, and administrative staff to ensure coordinated patient care.

Participate in clinical meetings, MDT discussions, and practice development activities.

Communicate effectively with patients, carers, and colleagues, demonstrating empathy and professionalism.

Ensure accurate coding and documentation of clinical activity.

Assist with recall systems for screening, immunisations, and chronic disease reviews.

Support the practice in meeting contractual and regulatory requirements.

Uphold confidentiality and data protection standards at all times.

Contribute to a positive, supportive working environment.

Undertake any other duties appropriate to the role as requested by the practice management team.

Work autonomously within professional boundaries while recognising when to seek support.

Adhere to NMC standards, professional codes of conduct, and practice policies.
