Job summary

We are currently recruiting for two positions, with up to a total of 60 hours per week available. We are happy to consider a full-time equivalent (FTE) position and one part-time position, or alternatively, as two part-time roles, depending on candidate availability and service requirements.

SAPA 5 is a Primary Care Network located in North Sheffield, made up of six practices within the S5 postcode area. We work in partnership with community services, mental health teams, social care, pharmacy, hospital providers, and the voluntary sector to deliver proactive, personalised, coordinated, and integrated health and social care for local residents closer to home.

The successful candidate will work closely with our established clinical teams to deliver high-quality patient care in an innovative and collaborative way. This role offers an excellent opportunity to further develop clinical skills within a positive, supportive, and excellence-focused environment.

Main duties of the job

The post holder will be a qualified clinical pharmacist who works within their professional boundaries while supporting and collaborating with our general practice teams. In this role, they will receive guidance and mentorship from a designated clinical supervisor.

The post holder will offer primary support to general practice staff by managing prescription and medication-related queries. They will contribute to the repeat prescription process, handle acute prescription requests, and carry out medicines reconciliation during transfers of care. In addition, they will help maintain safe prescribing systems and provide expert clinical medicines advice, ensuring the public and social care needs of patients within the GP practices are met.

  • Experience of using Emis Web.
  • Experience of using TPP SystmOne.
  • Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
  • Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice.

  • Independent Prescriber qualification
  • Strong communication skills
  • Able to work flexibly across practices and within a multidisciplinary team
