Job summary
We are currently recruiting for two positions, with up to a
total of 60 hours per week available. We are happy to consider a full-time
equivalent (FTE) position and one part-time position, or alternatively, as two
part-time roles, depending on candidate availability and service
requirements.
SAPA 5 is a Primary Care Network located in North Sheffield,
made up of six practices within the S5 postcode area. We work in partnership
with community services, mental health teams, social care, pharmacy, hospital
providers, and the voluntary sector to deliver proactive, personalised,
coordinated, and integrated health and social care for local residents closer
to home.
The successful candidate will work closely with our
established clinical teams to deliver high-quality patient care in an
innovative and collaborative way. This role offers an excellent opportunity to

Main duties of the job
The post holder will be a qualified clinical pharmacist who
works within their professional boundaries while supporting and collaborating
with our general practice teams. In this role, they will receive guidance and
mentorship from a designated clinical supervisor.
The post holder will offer primary support to general
practice staff by managing prescription and medication-related queries. They
will contribute to the repeat prescription process, handle acute prescription
requests, and carry out medicines reconciliation during transfers of care. In
addition, they will help maintain safe prescribing systems and provide expert
clinical medicines advice, ensuring the public and social care needs of
patients within the GP practices are met.
