Job summary
We are a three partner, two site, progressive practice in North
Sheffield looking to recruit a Practice Nurse to our friendly supportive team
across both sites.
We are looking for someone who can deliver a high standard of nursing
services to the practice population; including treatment, screening and advice
in accordance with the N.M.C. code of professional conduct. Working with
the GPs and nursing team to meet patients medical needs.
The successful candidate will have
excellent clinical skills and be dedicated to patient care. They will be
friendly and approachable in order to integrate with our close knit team. They
will be focussed and reliable and be willing to work collaboratively and
independently as required by the clinical scenario.
Experience in COPD and asthma management as
well as spirometry would be a definite plus but we are happy to provide full
training/access to appropriate courses.
We're looking for someone enthusiastic to
complement our established team.

Main duties of the job
Purpose
To deliver a high standard of nursing service to the practice population; including treatment, screening and advice in accordance with the N.M.C. Working with colleagues to provide core nursing care.
Key Responsibilities:
- Adhere to all practice policies and procedures.
Chronic disease management for conditions such as Asthma, COPD and Diabetes:
- Monitor and maintain patient registers and targets
- Perform audits
- Assist in maintaining the practice protocols.
Nursing duties including:
- Wound care
- Ear Syringing
- Adult and childhood immunisations
- Travel advice and immunisations
- Cervical cytology
- Contraception Advice
- Advice on general health and minor ailments
- Home visits to housebound patients.
Record Keeping
Document the assessment of need and delivery of care, for each patient seen.
Treatment Room Duties:
- Ensure that Infection Control procedures are adhered to.
- Ensure the Treatment rooms are kept clean and tidy.
- Manage vaccine ordering, including cold chain storage, distribution and monitoring.
- Ensure all medical equipment is maintained, regularly serviced and any faults reported.
Education and Training
- Maintain NMC registration.
- Maintain the PREP standard, ensuring that you are aware of up-to-date research based evidence to support your role.
- Annual appraisal.
- Participate in training for yourself and others.
- Attend practice meetings.
About us
Our Practice team consists of 3 GP Partners, 1 Salaried GP, 2 Practice
Nurses, 1 HCA and 1 Phlebotomist alongside our friendly and supportive
administrative and reception team.
We meet every week for a variety of clinical, educational and team
meetings. We feel this is very important for both our education and
support of all the members of the team. In addition to this we have
monthly admin meetings for both clinicians and reception staff to keep us
up-to-date with the ever changing processes within Primary Care.
We are a training practice which keeps us thoroughly up-to-date, we use
SystmOne clinical systems and achieve consistently excellent QOF results.
We care for just over 5800 patients which gives us a small 'family'
feel and allows us to genuinely get to know our patients and provide true
continuity of care over time.

- Performing audits on the above.
- Assist in maintaining the practice protocols on all of the above.
Record Keeping
Document the assessment of need, delivery of care, the evaluation of the outcome of care and make changes to the care plan for the patient, ensuring that:-
- An appropriate entry is made in the patient medical records both paper and computer records.
- Appropriate forms are completed as necessary.
General Treatment Room Duties:
- Ensure that Infection Control procedures are adhered to.
- Ensure that instruments are cleaned, sterilised and stored in accordance with infection control policy.
- Ensure the Treatment rooms are kept clean and tidy.
- Manage, order and restock medical supplies in the treatment rooms and consulting rooms.
- Manage vaccine ordering, including cold chain storage and distribution, and fridge monitoring.
- Ensure all medical equipment is properly cared for, regularly services and any faults reported as soon as possible.

- Input of clinical data obtained during a consultation, including immunisations, BP etc.
- Retrieval of relevant medical information from the patient database for the purpose of medical audit or practice research.
