Advanced Nurse Practitioner (30 hours per week)

Job summary

We are looking for an enthusiastic Advanced Nurse Practitioner to join our friendly and supportive team at Welton Family Health Centre.

This is a full-time position (30 hours per week, Monday to Friday). Responsibilities include consulting with adults and children, some triaging of patient requests via our online consultation tool, conducting home visits as needed, and managing minor illnesses, and chronic conditions. Candidates should be confident in history taking, assessment, examination, and diagnosis, demonstrating evidence-based clinical decision-making and critical thinking.

Main duties of the job

  • Conduct comprehensive physical examinations, telephone assessments and clinical assessments.
  • Contribute to triage of clinical requests, helping manage demand efficiently.
  • Diagnose and manage a wide range of acute and chronic health conditions.
  • Prescribe medications within scope of practice and relevant prescribing authority.
  • Order and interpret diagnostic tests such as blood work, X-rays, ECGs, etc.
  • Initiate and manage care plans, including referrals to secondary or specialist care.
  • Provide health education and counselling to patients and their families.
