Job summary

12 months Maternity Cover with a possible option for ongoing employment.

The Clinical Pharmacist role has become an essential part of the management of patients within Primary Care and we are looking for an additional team member to join our team of pharmacists.

You can be an experienced or newly qualified Pharmacist where you will be delivering outstanding patient care within our organisation.

You will be supporting the team to optimise clinical safety within the organisation, develop clinical management plans, carry out comprehensive medication reviews, optimise long term condition outcomes, and deal with minor illness through remote and Face to Face consultations. You will furthermore help to improve clinical coding and data entry quality across the organisation. You will be supported clinically with your development into the role.

Main duties of the job

The post holder will work as part of a multi-disciplinary team in a patient-facing role, within the GP practice. In this role they will be supported by a clinical lead GP who will develop, manage and mentor them. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews and a variety of other tasks as outlined below. The post holder will be supported to develop their competencies and will take on additional roles as they gain more experience.
