Towcester Medical Centre is seeking an experienced and motivated Advanced Clinical Practitioner to join our friendly and supportive multidisciplinary team.
We are looking for an Independent Prescriber from a Nursing or Paramedic background with significant experience in primary care or a relevant clinical setting. The successful candidate will provide high-quality, patient-centred care within GP practice, including initial history taking, comprehensive clinical assessment, diagnosis, treatment, prescribing, and ongoing evaluation of care.
The role offers the opportunity to work autonomously within an established clinical framework, while benefiting from the support and collaboration of our experienced GP team when required. Main duties of the job
Attend to patients presenting with a wide range of acute and chronic medical conditions, providing expert clinical assessment and advice.
Undertake comprehensive history taking, clinical examination, diagnosis, treatment, and ongoing monitoring of patients within the Primary Care setting.
Deliver high-quality patient care for the management of long-term and chronic conditions in line with local and national guidelines.
Maintain accurate, timely, and contemporaneous clinical records using appropriate SNOMED coding.
Support the wider clinical team in the management of clinical and medical emergencies.
Practice at all times in accordance with the relevant professional code of conduct and scope of practice.
Prescribe independently within areas of clinical competence and in line with current legislation and best practice.
Initiate, evaluate, and modify a range of clinical interventions, including prescribing medications, recommending therapies, providing lifestyle advice, and coordinating ongoing care.

We have just over 11,700 patients, 5 GP partners, 3 salaried GPs, an experienced nursing team of 3 which includes 2 HCAs and strong administrative support. This gives us a strong basis to provide excellent patient care.

Assess and manage all patients presenting with minor illness, in line with practice policy and clinical guidelines.
Work collaboratively with GPs and the wider multidisciplinary team to ensure coordinated, high-quality patient care.
Take an active role in clinical supervision, mentorship, and reflective practice to support ongoing professional development.
Deliver chronic disease management clinics as required, in line with national and local guidelines.
Refer patients to secondary care services appropriately, following agreed referral pathways.
Maintain accurate, timely, and comprehensive clinical records, including the use of SNOMED coding.
