Job summary
To plan care for Practice patients to include, first contact, diagnosis
assessment, treatment, preventative care, chronic disease management, screening
and patient education.
Main duties of the job
Respiratory Care
To manage newly diagnosed patients with asthma and COPD.
To work within agreed local/BTS guidelines to manage patients with
asthma, aiding diagnoses, reviewing inhaler technique, advising on treatment
changes and encourage self-management.
Cardiovascular
Disease and Heart Failure
Develop and maintain a Practice register for patients
with heart failure and for secondary prevention of CHD. Work with patients to set action plans for changes of lifestyle.
Work within agreed protocols when managing risk factors to enable patients to
manage their own disease and provide psychological support by discussing
interventions and treatments. Attend CHD
lead meeting at the Primary Care Trust (PCT) and communicate with other
involved staff as necessary.
Diabetes and Family
Planning
Womens Health
To advise patients on the national and local cervical
cytology and mammography screening programmes, undertake cervical cytology
sampling and promote breast self-awareness.
9. Professional development
Maintain
continued education in accordance with the personal development plan agreed by
the Practice and the standards set by the NMC.
This will include attendance at courses and study days as well as
in-house training.

templates to collect QOF data from within QOF disease areas when assessing,
diagnosing and treating patients within agreed nursing protocols.

health surveillance and therapeutic interventions within a broader health
promotion/public health strategy.
e. To manage programmes of care for patients with
acute and chronic diseases by planning, providing and evaluating care. To prescribe
medicines for these patients as appropriate and within accepted parameters for
nurse prescribers.
