We are currently recruiting for 2 full time Care Coordinators for St Chads & Chilcompton Surgery and Somerton House Surgery
37.5 hours per week - £12.25 per hour
Main duties of the job
The successful candidates will join an established group of Care Coordinators, Social Prescriber Link Workers and other health care professionals to support:
St Chads & Chilcompton Surgery and Somerton House Surgery
You will ensure that patients receive the best care possible. Coordinating the work of healthcare professionals and non-clinical staff, you will ensure the provision of an effective patient-focused service.
Your role is a key part of our growing and progressive PCN, working to provide an enhanced care experience, with a view to improving the service.
We are seeking an enthusiastic, patient-focused, and highly motivated individuals to work within our innovative and supportive organisation alongside GPs, Nurses, First Contact Physiotherapists, Social Prescribers and Pharmacists as part of our multi-disciplinary team.

Essential
- Good general standard of education equivalent to or GCSE A-C
- Motivated to help people
- Experience of working within Health care supporting vulnerable groups
- Enthusiastic , caring and empathetic
- Competent IT skills
Desirable
- Level 2 Diploma in Health and Social Care
- Experience of using System One
- Have familiarity with local resources and services and how to access them
