Practice Nurse with Diabetes Experience

Job summary

We are seeking a dedicated and knowledgeable Practice nurse with diabetic experience to join our healthcare team. The successful candidate will play a vital role in providing specialised care and education to patients managing diabetes. This position offers an excellent opportunity to utilise your expertise in patient care and anatomy, contributing to improved health outcomes for individuals living with diabetes. The role requires a compassionate approach, strong communication skills, and a thorough understanding of diabetes management and related anatomical concepts.

Main duties of the job

You will play a key role in delivering evidence-based diabetes care in line with QOF, NICE guidance, and local pathways. Working independently and collaboratively with GPs, pharmacists, Nurses, HCAs, and the wider MDT, you will support our diabetic population through structured reviews, patient education, and personalised care planning. You will also be expected participate in other practice nurse roles

To be responsible for the delivery of practice nursing services, working as part of the practice team, delivering care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management and health prevention.

Responsibilities .

  • Deliver comprehensive diabetes clinics, including annual reviews and interim follow-ups
  • Optimise diabetes medication
  • To provide wound care (ulcer/Doppler etc.) to patients, ECG, Venepuncture, Manual BP, lifestyle reviews
  • To implement vaccination programmes for adults and children
  • To support the clinical team with all safeguarding matters in accordance with local and national policies
  • Provide individualised patient education and self-management support
  • Interpret and act on diagnostic tests (e.g., HbA1c, lipids, kidney function)
  • Work closely with the practice team to optimise QOF outcomes and reduce health inequalities

As a Diabetes Nurse, you will play a key role in managing and supporting patients with diabetes, working independently as well as part of a team to deliver a high standard of patient care. Your role will involve providing comprehensive diabetes management, including education, monitoring and treatment to patients. You will also support patients with prevention strategies, lifestyle changes and initiate treatment for newly diagnosed patients.

Diligence in record keeping.

Working as part of a team involving doctors, nurses, receptionists and treatment room staff.

Maintain clinical knowledge and be motivated to pursue independent study in line with perpetually evolving clinical guidelines.
