Job summary

An excellent opportunity has arisen to join our friendly and established Data Team.

This is a, mainly administrative, role responsible for the scanning and read-coding of medical information into the patients electronic healthcare record, summarising new patient medical records and being part of the recalls process for long term conditions. The successful candidate will need to have excellent attention to detail, good IT skills, work well both in a team and under own initiative and have a positive can do attitude. We offer a competitive salary, hours open to negotiation, Sovereign Healthcare Asset L2 which includes Store discounts, NHS Pension and 25 days holiday plus bank holidays.

In addition, the post-holder will also support the administration team, providing administrative support as required, in line with the needs of the practice. Hybrid working between home and office would be considered for the right candidate once fully trained and competent in the role. This would be 2 days at home and 3 days in the office weekly.

30-37.5hrs per week will be considered. Previous applicants need not apply.

Main duties of the job

Accurately read-coding data on SystmOne into the patient record. Identifying relevant clinical information documented in patient correspondence, coding appropriately in patient records, completing any detailed actions and forwarding documents on to other teams where required.

Accurately summarising new patient medical records onto SystmOne as per policy.

Assisting the recalls team to ensure patients with long term conditions are invited to appointments in a timely manner and follow up appropriately; this also links in with our Qof register to ensure the practice receives correct payments.

Job description

Job responsibilities

The following are the core responsibilities of the Data Administrator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Accurately read-coding data on SystmOne into the patient record

b. Identifying relevant clinical information documented in patient correspondence, coding appropriately in patient records and completing any detailed actions

c. Being aware of Qof requirements when doing the above

d. Record sensitive information accurately eg HIV status, child protection register, safeguarding issues

e. File and store records as required

f. Photocopy documentation as required

g. Action all incoming faxes, emails and mail in a timely manner

h. Scanning of patient related documentation and attaching scanned documents to patients healthcare records

i. Input data into the patients healthcare records as necessary

j. Reviewing records and accurately producing a summary of the patients medical history

k. Reviewing progress of summarising ensuring practice targets are achieved

l. Maintain a clean, tidy, effective working area at all times

m. Support all clinical staff with general administrative tasks as requested

n. Full management of the long-term conditions reporting

o. Maintain a good working knowledge of current QOF and recall processes and requirements.

p. Administer the patient invitation process for long-term condition reviews and other appointments as required.

q. Monitoring patient recall and recall systems.

r. Ensure accurate report output.

s. Dealing with queries via email, telephone and SystmOne tasks relating to Long Term Conditions patients and recalls

t. Ensuring patient records are updated accordingly.

u. Liaising with the practice clinical team

v. Liaising with patients, relatives, hospitals and other agencies
