Role Overview:As a Practice Nurse, you'll play a vital role in the delivery of chronic disease management and general treatment room services. A particular interest or specialism in diabetes is highly desirable, though a broad knowledge across all chronic conditions (including Asthma, COPD, and CHD) is preferred. Training and support will be available and expected where upskilling is required.

- Chronic disease management (including Diabetes, Asthma, COPD, CHD)
- Childhood immunisations
- Cervical cytology and smear tests
- Supporting Enhanced Access sessions as part of our extended hours offering

- Patient Base:17,500 patients
- Team Environment: Strong collaborative working across GPs, nurses, and our on-site community team; weekly MDT meetings
- Clinical Services: CQC-rated Good, high QOF performance, in-house minor surgery, LARC clinics, on-site Physio and pharmacy support
- Systems & Development: EMIS Web, regular team meetings (SEAs, safeguarding, palliative care, clinical discussions)
- Specialty Clinics: Dedicated clinics for CHD, LVSD, COPD, and diabetes; full sexual health provision

- Essential Criteria:
- Registered Nurse (NMC registration)
- Previous experience working as a Practice Nurse or in a primary care setting
- Confident in delivering general treatment room services.
- Knowledge and experience in managing long-term conditions (e.g. asthma, COPD, CHD, Diabetes)
- Excellent communication and interpersonal skills
- Able to work independently and as part of a multidisciplinary team
- Proficient IT skills and experience using clinical systems (e.g. EMIS Web or equivalent)
- Commitment to ongoing professional development and willingness to undertake further training
Desirable
- Desirable Criteria:
- Special interest or qualification in diabetes management
- Experience running or supporting chronic disease clinics, particularly for diabetes
- Experience contributing to QOF achievement
- Knowledge of Enhanced Access services and flexible to work during extended hours if required
- Familiarity with clinical governance processes (e.g. SEAs, audits)
- Evidence of continued professional development in chronic disease or primary care nursing
