Job summary
We are
looking for a full-time (35 hours) Care Navigator/Receptionist to join our
friendly, patient-focused team.
This
position is a permanent one and working hours will be over 5 weekdays, shift
patterns to be confirmed.
Main duties of the job
The
successful applicant will be:
-
Experienced with SytmOne -Minimum
of 1 year experience and must be able to demonstrate where this experience was obtained.
-
Organised, efficient and
demonstrate good interpersonal skills and effective communication within our
multi-professional Primary Care Team.
-
You will need to have the
capability to prioritise your own workload, be confident and adaptable to both
change and to challenging situations in a busy environment.
-
You will also be able to
demonstrate co-operative and productive working with people of all backgrounds
and second language would be desirable for this role.

You will need to computer
literate, experienced in the use of electronic communications and be confident
during face to face interactions with patients.

show attention to detail even under pressure while maintaining confidentiality
and the need to be able to deliver accurate and clear information to
appropriate parties, in a timely manner.

administration duties.

To embed and sustain an ethos of care navigation throughout the
team; providing advice and guidance and signposting patients to the most appropriate
clinician/service.
