Responsibilities
Core responsibilities for HCA. There may be a requirement on occasion to
carry out additional tasks, this will be dependent of staffing levels, workload,
appropriate training, and experience.
Support the clinical team by contributing to the delivery of
health promotion programmes, including patient education, assisting in long
term condition management programmes, assisting with delivery of direct patient
care, within personal scope and limitations of practice and administrative
tasks as required to support these areas.
This will include, but is not limited to:
Health promotion
and education
Baseline observations of height, weight, BMI,
pulse, smoking and alcohol consumption, with knowledge of normal levels and
guidelines
Weight management
Blood pressure monitoring and HBPM
Blood glucose monitoring and education for home
monitoring
NHS health checks
Smoking cessation
Diabetes prevention
Opportunistic health promoting as appropriate
Long term
condition management
Blood pressure and hypertension monitoring
Diabetes
Learning disabilities
Mental health
Assisting
clinicians
Venepuncture
Wound care
Administration of injections under PSD
ECG recording
Chaperoning
Administration
Referrals as appropriate for the above
Encourage uptake of screening and vaccination
programmes
Infection control, including personal
responsibilities and support for practice IC lead
Stock management and ordering of supplies,
including rotation to avoid waste and oversight of vaccine fridge monitoring
Prescription reconciliation for practice stock
Administration to support delivery of practice
LTC management and health promotion programmes
Participate in practice nurse team meeting and
contribution to developing of this service
Support of new practice team members
Participate in local and national health
initiatives to enhance service delivery, patient care and wellbeing
Confidentiality
In the course of seeking treatment, patients
entrust us with, or allow us to gather, sensitive information in relation to
their health and other matters. They do
so in confidence and have the right to expect that staff will respect their
privacy and act appropriately
In the performance of the duties outlined in
this job description, the post-holder may have access to confidential
information relating to patients and their carers, practice staff and other
healthcare workers. They may also have
access to information relating to the practice as a business
organisation. All such information from any source is to be regarded as
strictly confidential
Information relating to patients, carers,
colleagues, other healthcare workers or the business of the practice may only
be divulged to authorised persons in accordance with the practice policies and
procedures relating to confidentiality and the protection of personal and
sensitive data
Health
& safety
The
post-holder will assist in promoting and maintaining their own and others
health and safety and infection control as defined in the practice Health &
Safety policy, the practice Health & Safety manual, and the practice
Infection Control policy and published procedures. This will include (but will
not be limited to):
Using personal security systems within the
workplace according to practice guidelines
Identifying the risks involved in work
activities and undertaking such activities in a way that manages those risks
Responsible for correct hand hygiene of self
Keeping own work areas and general/patient areas
generally clean, assisting in the maintenance of general standards of
cleanliness consistent with the scope of the post holders role.
