Job responsibilities

Responsibilities

Core responsibilities for HCA. There may be a requirement on occasion to carry out additional tasks, this will be dependent of staffing levels, workload, appropriate training, and experience.

Support the clinical team by contributing to the delivery of health promotion programmes, including patient education, assisting in long term condition management programmes, assisting with delivery of direct patient care, within personal scope and limitations of practice and administrative tasks as required to support these areas.

This will include, but is not limited to:

Health promotion and education

Baseline observations of height, weight, BMI, pulse, smoking and alcohol consumption, with knowledge of normal levels and guidelines

Weight management

Blood pressure monitoring and HBPM

Blood glucose monitoring and education for home monitoring

NHS health checks

Smoking cessation

Diabetes prevention

Opportunistic health promoting as appropriate

Long term condition management

Blood pressure and hypertension monitoring

Administration of injections under PSD
