Main duties of the job

To purpose of the Nurse Associate is to work strictly in accordance with specific practice guidelines and protocols. The Nurse Associate will assist the Practice Clinical Team in the provision and delivery of prescribed programmes of patient care.

Aspects of work include:

Delivering evidenced based, person-centred care to patients

Providing general and specific health screening to the practice patients (within agreed protocols) with referral to other clinical colleagues as necessary

Undertaking primary care nursing functions, such as

Cervical screening

Health promotion activities

Long term disease management (asthma, COPD, diabetes)

Wound care and compression, including assessment and care planning

Medicines Administration (B12, Flu, Shingles, Pneumonia, Depo injections)

Job description

Job responsibilities

KEY DUTIES

  • To deliver evidenced based, person-centred care to patients
  • To provide general and specific health screening to the practice patients (within agreed protocols) with referral to other clinical colleagues as necessary
  • To undertake primary care nursing functions, such as cervical screening; health promotion activities,  long term disease management (asthma, COPD, diabetes), wound care and compression, including assessment and care planning, medicines administration (B12, Flu, Shingles, Pneumonia, Depo injections)
  • Recognise issues relating to safeguarding vulnerable children and adults and report any problems or raise concerns to the appropriate registered care
  • Demonstrate good understanding of principles of consent and ensure valid consent is obtained prior to undertaking nursing and care procedures

OTHER DUTIES

  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
  • To use Arden’s templates where appropriate and ensure all consultations are recorded at the time of event with accuracy and due diligence.
  • To act with professionalism and ensuring that ethical conduct is adhered to at all times
  • To keep the working area clean, tidy and professional, adhering to infection control guidance and GDPR regulations
  • To save and discard patients appropriately (not to leave patients records open when unattended) and record patient consent
  • To assist with training for new staff
  • To assist with the gathering of statistics and information when required and participate and co-operate with any research projects
  • To provide cover for members of the team during periods of sickness and annual leave

ADMINISTRATION

  • Contributes and participates in audits, evaluation and clinical standard setting within the Practice and/or PCN.
  • Accurate and timely summarising of patient records and read-coding patient data
  • Complete all required paperwork for legal and administrative purposes in accordance with relevant standards
  • Ensure that all practice policies are fully implemented

Work in accordance with all governance and internal systems relating to (but not limited to) the management of clinical data and systems

TRAINING AND DEVELOPMENT

  • Taking responsibility for own development with relevant evidence-based knowledge and competence in all aspects of the role to meet clinical governance guidelines for Continuing Professional Development (CPD) and a Personal Development Plan (PDP).
  • Stay up to date through attendance at any courses and/or study days necessary to ensure that professional development requirements are met, demonstrating skills and activities to others who are undertaking similar work.
  • Subject to a performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
  • Work closely with other clinical staff and administrative managers in the setting up and/or improving of practice systems for monitoring/measuring performance against Clinical Governance and Quality Indicator targets

  • Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures

  • Take part in the maintenance of quality governance systems and processes across the Practice and its activities.
  • Utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required.
  • Work with others on improving the quality of healthcare in response to local and national policies and initiatives as appropriate
  • Evaluate patients’ response to health care provision and the effectiveness of care
  • Support and participate in shared learning across the practice and wider organisation
  • Manage, review and identify learning from patient complaints, clinical incidents and near-miss events
