Job summary
Beech Tree Surgery are looking to recruit an Advanced Clinical Practitioner to join our multi-disciplinary clinical team to provide a high standard of care for our practice population of approximately 16,500.
We are a friendly, supportive practice with a strong ethos of working with kindness, honesty and transparency. We are a teaching practice with experience in training many different roles across the practice.
Main duties of the job
This role will be a mix of face to face telephone consultations and home visits when required. We encourage applicants with specialist interests who wish to expand on this in collaboration with us.
The successful applicant will demonstrate critical thinking skills and have the ability to follow practice polices and procedures. They will work in a collaborative way with colleagues throughout the practice to meet the needs of our patients with high quality.
About us
We are a 4 Partner GP practice situated in a purpose-built premises in Selby providing care to 16,153 patients. We also operate two part time branch surgeries in the villages of Riccall and Carlton.
We have a close practice team who are friendly, approachable and supportive. We strive to provide a high standard of care and work closely with partnerships and PCN.

- Care for patients presenting with a range of conditions from routine to acute and chronic medical conditions.
- Implement and evaluate treatment plans for patients
- Identify and manage patients at risk of, and diagnosed with long term conditions
- Provide advanced care to patients as required in accordance with clinical based evidence (NICE & NSF)
- Requesting of pathology services and interpreting results
- Maintain accurate clinical records
- Assist in managing chronic disease registers
- Support patients in the use of their medications, reviewing annually when required
- Contribute to practice targets
- Liaise with external services to appropriately support patients
- Delegate clinical responsibilities appropriately
- Support the team with all safeguarding matters in accordance with local and national policies
- Deliver health promotion where appropriate
