Job summary

An exciting new opportunity has arisen at Selby Town Primary Care Network (PCN) for a Children & Young Persons Neurodiversity Care Coordinator to join our friendly and dynamic Learning Disabilities & Neurodiversity team.

The aim of this role is to improve the patient experience from pre diagnosis to post diagnosis and throughout their neurodiversity journey. It will encompass children and young people between the ages of 5 and 21 years of age who have or are awaiting a diagnosis of a neurodiverse condition such as Autism and ADHD.

Pivotal to this role is ensuring patients are identified, discussed & treated to evidence-based standards in accordance with agreed care plans. In particular, it will raise the proactive input from Primary Care at all stages & provide a clear & accessible point of contact for guidance, support & navigation. This project will be fully integrated into local service delivery, encompassing the contributions from health, social care & voluntary agencies. An integral part of the role will also be working with families/carers to ensure they are informed, understood, and supported.

About us

The award-winning Selby Town PCN consists of four practices with a combined population of approximately 50,000 patients, covering the large geographical area surrounding Selby and Escrick.

This is a post supporting patients & staff across the four practices in the PCN: Beech Tree Surgery, Escrick Surgery, Posterngate Surgery and Scott Road Medical Centre.

The PCN has a Clinical Director, a Clinical Lead GP and is managed by the PCN Operations Manager. We work in increasingly collaborative partnerships across a number of workstreams, which include important priority areas of mental health, frailty, urgent care and health inequity.

Our team consists of a diverse multi-professional workforce which includes pharmacists, MSK practitioners, physician associates, mental health workers, care coordinators, social prescribing link workers, a frailty therapy team, an urgent care coordinator, a business intelligence analyst/digital lead, a transformational lead and an urgent care team of health care professionals.

KEY RESPONSIBILTIES

1. Data gathering & organization:

  • Developing an understanding of our coding of neurodiverse conditions using clinical templates
  • Identifying missed recordings of diagnosis due to coding errors.
  • Developing and maintaining a register of patients with specific neurodiverse conditions
  • Monitoring systems that inform GPs about secondary care episodes, outpatient/inpatient & Emergency Department attendances, as well as Out of Hours activity.

2. Develop & reviewing care plans to include:

  • Being involved in the introduction of & changes to care plans
  • Ensuring care plans & advanced care plans are up to date & shared with the relevant people.
  • Ensuring that all patients are discussed & outcomes are recorded & reviewed.
  • Collaborating with staff teams to ensure any planned or recommended treatment occurs in a dependable & timely manner acting as a single point of contact.
  • Informing the Lead Clinician of any delays in assessment or delivery of care needs. Identify where plans or recommendations are not achieved & undertake process mapping to identify any bottlenecks.

3. Develop communications & effective reporting to include:

  • Negotiating with all involved in care issues to establish & promote good working relationships & improve the coordination of the patient journey.
  • Establishing effective communication networks with clinicians & other team members & ensuring the continuation of effective communication. Developing close working relationships with the existing Care Coordinators & Wellbeing Link Workers
  • Recognising peoples needs for alternative methods of communication & respond accordingly.
  • Preparing reports on behalf of Clinical Leads for peer review activities.
  • Updating & maintaining a Directory of Services in real time.
