Main duties of the job
Support to general practice staff
with regards to prescription and medication queries
Medicines reconciliation on transfer
of care and systems for safer prescribing
Expertise in clinical medicines
advice while addressing both public and social care needs of patient in the GP
practice
clinical leadership on medicines
optimisation and quality improvement

- Completed or enrolled in CPPE Pathway
- Able to run chronic disease clinics
- Offer Medicines advice to patients and clinical staff
- Undertake audit activities and discussions with the General Practice team
- Have a Masters in Pharmacy and a minimum of 2 years post reg experience
- Independent Prescribing Pharmacist: 1 year (required)

- Completed or enrolled in CPPE Pathway
