Job summary
We are looking for a proactive and forward-thinking Practice Nurse to join our team. You will be joining a friendly and supportive team, and will be supported by the GP partners and practice management team to ensure that we are able to deliver outstanding care to our patients. This is a part-time role (30 hours per week). Wednesday, Thursday, and Friday will be in practice. Either Monday or Tuesday will be worked from home, completing admin tasks and telephone calls.
Main duties of the job
The role will involve management of our nursing team, leading the team, responding to changes in nursing practice.
Clinical responsibilities will include:
- Assess, plan, develop, implement and evaluate programmes which promote health and well being, and prevent adverse effects on health and well being.
- Assess, plan, develop, implement and evaluate individual treatment plans for patient with a known long term condition
- Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition as appropriate.
- Lead on wound care management, participate in immunisation programmes for both adults and children.
- Infection control.
About us
We are a small and friendly GP surgery, based on the outskirts of Chester city centre. We provide care for 8,000 patients and are led by 4 supportive GP partners. Our practice team also includes 2 salaried GPs, 3 ANPs, 1 additional Practice Nurse, 3 HCAs, physiotherapist, dietitian, child and adult mental health practitioners, care co-ordinator, and a large administration team.

- Identify, assess and diagnose patients with complex health needs across the practice population
- Lead the introduction and use of technical and specialist nursing treatments for patients with complex healthcare needs
- Prescribe products to meet the needs of patients within the practice population
- Lead and develop programmes to improve health and well-being and meet the specific needs of communities, groups and individuals
- Work with others to change policies and improve health and well-being and reduce inequalities
- Lead the implementation and evaluation of evidence-based practice to improve the services offered by the practice nursing team
