This is a key clinical role within our same day and routine care teams. You will work autonomously, supported by a strong MDT including GPs, ACPs, nurses, pharmacists, and an excellent administrative and management team.

Main duties of the job

As an Advanced Nurse Practitioner, you will:

  • Provide high quality clinical assessment, diagnosis, treatment, and evaluation for a wide range of presentations
  • Manage urgent/same day demand and contribute to the smooth running of the practice

  • Support long-term condition management and proactive population health initiatives
  • Undertake clinical examinations, investigations, and prescribing within your scope of practice
  • Work collaboratively across our sites to ensure continuity and excellent patient experience
  • Contribute to clinical governance, audit, and quality improvement
  • Offer leadership, mentorship, and support to colleagues where appropriate

Job description

Job responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Provide a home visiting service

b. Provide weekly ward rounds to allocated care homes

c. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients

d. Implement and evaluate individual specialised treatment plans for chronic disease patients

e. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

f. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

g. Request pathology services as necessary

h. Process and interpret pathology and other test results as required

i. Maintain accurate clinical records in conjunction with extant legislation

j. Ensure read codes are used effectively

k. Maintain chronic disease registers

l. Prioritise health issues and intervene appropriately

m. Support the team in dealing with clinical emergencies

n. Recognise, assess and refer patients presenting with mental health needs

o. Extended and supplementary prescriber, adhering to extant guidance

p. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

q. Contribute to practice targets (QOF etc.), complying with local and regional guidance

r. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

s. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

t. Support the clinical team with all safeguarding matters, in accordance with local and national policies

u. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

v. Deliver opportunistic health promotion where appropriate
