RESPONSIBILITIES & ACCOUNTABILITIES
Clinical responsibilities:
- In accordance with the clinical rota, as agreed, the
post-holder will make themselves available to undertake a variety of
duties, including surgery consultations, telephone consultations and
queries, visiting palliative patients at home and care homes, prescriptions and dealing with queries, paperwork and correspondence in a
timely fashion
- Making professional, autonomous decisions in
relation to presenting problems, whether self-referred or referred from
other health care workers within the organisation
- Assessing the health care needs of patients with
undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and
early signs of illness
- Developing care plans for health in consultation
with patients and in line with current practice disease management
protocols
- Providing counselling and health education
- Admitting or discharging patients to and from the
caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation
notes to agreed standards
- Compiling and issuing computer-generated acute
and repeat prescriptions
- Prescribing in accordance with the practice
prescribing formulary (or generically) whenever this is clinically
appropriate
- In general, the post-holder will be expected to
undertake all the normal duties and responsibilities associated with a GP
working within primary care.
Other responsibilities

- Awareness of and compliance with
all relevant practice policies
- Contribute to the evaluation of the clinical
standards within the organisation including participating in audits and
provision of evidence based results.
- Assist in maintaining a wide skill mix within the
team, ensuring a range of service delivery.
- Contributing to the development of computer-based
patient records
- Contributing to the summarising of patient
records and read-coding patient data
- Attendance at practice/ partnership, clinical,
MDT and educational meetings as well as external events as appropriate.
- Commitment and contribution to achievement of the
highest possible standards such as QOF, Enhanced Services, Prescribing and
other performance targets.
- Active participation in the management and development
of the Practice.

The
post holder would be expected to work with the following practice policy

Communication:
Use and
demonstrate sensitive communication styles, ensuring patients are fully
informed and consent to treatment
- Communicate
effectively with patients and carers, recognising the need for alternative
methods of communication to overcome different levels of understanding,
cultural background and preferred ways of communicating
- Communicate with other
healthcare professionals including the referral of patients to secondary
care
- Encourage effective
communication within the practice environment, team members and with
external stakeholders
- Ensure awareness of
sources of support and guidance and provide information in an acceptable
format to all patients, recognising any communication difficulties
Make improved use of the
telephone and other technologies
Delivering a quality
service:
- Produce accurate,
contemporaneous and complete records of patient consultation, consistent
with legislation, policies and procedures
- Prioritise, organise
and manage own workload in a manner that maintains and promotes quality
- Deliver care according
to NSF, NICE guidelines, practice policies and other evidence-based care
- Assess effectiveness
of care delivery through self and peer review, benchmarking and formal
evaluation
- Initiate and
participate in the maintenance of quality governance systems and processes.
- Evaluate patients
response to health care provision and the effectiveness of care
- Support and
participate in shared learning across the practice and wider organisation
- Ensure that where
appropriate, adverse incidents are recorded in line with the practice
policy and attend significant event meetings.
- Take part in practice
learning opportunities
- Assess the impact of
policy implementation on care delivery
- Be aware of statutory
child/vulnerable patients health procedures and local guidance and
practice with these guidelines
- Alert other team
members to issues of quality and risk
Leadership
personal/professional and people development:
In
addition to maintaining continued education through attendance at any courses
and/or study days necessary to ensure that professional development
requirements for PREP are met, the post-holder will participate in any training
programme implemented by the practice as part of this employment, with such
training to include:
Participation in
an annual individual performance review, including taking responsibility for
maintaining a record of own personal and/or professional development
Taking
responsibility for own development, learning and performance and demonstrating
skills and activities to others who are undertaking similar work.
Support staff
development in order to maximise potential
- Actively promote the
workplace as a learning environment, encouraging everyone to learn from
each other and from external good practice
- Critically evaluate
and review innovations and developments that are relevant to the area of
work
- Enlist support and
influence stakeholders and decision-makers in order to bring about new
developments in the provision of services
- Take a lead role in

development of practice guidelines, protocols and standards

- Undertake mentorship
for junior staff, assessing competence against set standards
