Job summary
The GP Practice serves a population of around 18,000 people and provides services such as routine and same day appointments, practice nurse appointments, phlebotomy and chronic disease management.
An opportunity has arisen to join the urgent care team in our large, friendly and forward thinking practice.
Ideally, you should have experience of working in a primary care setting or similar, have a prescribing qualification and enjoy working in a fast paced, multi-disciplinary team.
Main duties of the job
The successful candidate will act as an autonomous practitioner working independently and with other health care professionals to prioritise, diagnose and treat patients, primarily within the Urgent Care Clinics, who present with a range of acute, non-acute and chronic medical conditions, initiating direct referrals as appropriate. The post holder will need to collaborate with GPs, nursing staff and non-clinical support staff alike.
Willing to develop, trial and adopt new working methods designed to manage the increasing demand in primary care, by using skills and experience to contribute to the needs of the GP Practice as we progress for the overall benefit of our patients. This includes working as part of the triage team assessing and signposting patient requests as they arrive.
In order to work at this level, NMC requirements for advanced practice must be met.
About us
We have a team of advanced nurse practitioners and paramedic practitioners who work as part of the triage team. This team work alongside the GPs and care navigators to manage the health of the community and assign appropriate care pathways.

- Assess,

- Clinically
examine and assess patient needs from a physiological and psychological
