Practice Nurse

Job summary

We are looking for an experienced Practice Nurse to join our team of ANPs, Nurses and Health Care Assistants.You will work as part of the Nursing team, delivering chronic disease management while also responsible for childhood immunisations, cervical screening and travel vaccinations to name a few areas. You will bring ideas for quality improvement and streamline nursing services, and a passion for outstanding patient experience.

You will report to and work closely with our Nurse Lead and Clinical Team to support our patients in the best way possible.The role is for 20 hours a week.

We are a forward thinking training practice based in Belper. We look after over 12,000 patients and offer a full range of primary care services, based in a modern surgery.

Main duties of the job

  • ECGs
  • Venepuncture
  • INRs
  • New patient medicals
  • Chaperone procedure
  • Requesting pathology tests and processing the results, advising patients accordingly
  • Travel medicine
  • Diabetes
  • Hypertension
  • Asthma
  • COPD
  • Spirometry
  • CHD
  • Immunisations (routine, childhood and travel)
  • Womens health (cervical cytology, contraception, etc.)
  • Understand the importance of evidence based practice
  • Broad knowledge of clinical governance
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and understand when to refer to GPs
  • Understanding of infection prevention and control measures
  • Knowledge of health promotion strategies
  • Understand the requirement for PGDs and associated policy

Depending on experience, We will encourage you to support the training and development for nurses and health care assistants within the Practice. The successful applicant along with the Lead Nurse will contribute to continuous development of nursing services to deliver efficient, effective and outstanding patient services.

To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

To request pathology services as necessary

To maintain accurate clinical records in conjunction with extant legislation

To ensure read codes are used effectively

To chaperone patients where necessary

To prioritise health issues and intervene appropriately

To support the team in dealing with clinical emergencies

To help with supervision and support our HCAs

To support the clinical team with all safeguarding matters in accordance with local and national policies

To deliver opportunistic health promotion where appropriate
