General Practice Care Coordinator

Job summary

The Borchardt Medical Centre is seeking a proactive, organised, and patient-focusedCare Coordinatorto join our friendly General Practice team. This is an exciting opportunity to support our patients in accessing the right care at the right time, while helping to improve the continuity and quality of services across the practice and wider Primary Care Network (PCN).As a key member of our multidisciplinary team, you will help patients navigate health, social care, and voluntary sector services, support long-term condition management, and contribute to improving patient experience and outcomes. The role also includes undertaking phlebotomy and clinical filing/coding as part of patient care.

Main duties of the job

Patient Coordination & Navigation

  • Act as a central point of contact for patients needing additional support.
  • Coordinate appointments, investigations, and follow-up actions.
  • Help patients understand and access a wide range of services.
  • Support patients to attend and engage with their healthcare appointments.

Proactive Support for Long-Term Conditions

  • Identify patients who require reviews, such as frailty, chronic disease, and cancer care reviews.
  • Work with GPs, nurses, pharmacists, social prescribers, and wider PCN colleagues.

Administrative & Coordination Duties

  • Manage recall systems for long-term condition clinics.
  • Maintain accurate records on clinical systems (EMIS).
  • Prepare information for MDT meetings and follow up actions.
  • Support quality improvement and patient experience projects.

Phlebotomy

  • Perform venepuncture safely and in line with practice protocols.
  • Correctly label, record, and handle specimens.
  • Ensure patient comfort, identification, and informed consent.
  • Recognise and escalate concerns or adverse reactions.

Additional Responsibilities

  • Provide administrative support to clinicians and the Practice Manager, including delegating and following up tasks where appropriate.
  • Participate in mandatory training, induction, appraisal, and service development activities.
  • Undertake other reasonable duties aligned to the role.
