Practice Clinical Pharmacist

Job summary

We are seeking a motivated and experienced pharmacist to work directly with multidisciplinary teams to improve clinical effectiveness and person-centred outcomes.The successful candidate will play a key role in medicines optimisation, clinical medication reviews, and the safe and effective use of medicines across the practice population.

The practice population is diverse and varies in demographics, so the breadth of health care need is wide. This makes working in general practice interesting and no day is the same.The role will be based within the practice providing support to the GPs and other healthcare professionals within the teams to improve the health of patients through the rational and safe use of medicines.

The post holder will contribute to the efficient working of the practice team by leading on key areas in relation to medicines optimisation. The role will include face to face and remote patient contact, as well as liaison with colleagues in community pharmacy, care homes and secondary care.

Main duties of the job

  1. To develop and facilitate a good working relationship with all local providers of healthcare.
  2. To liaise effectively with other health care teams concerned with the patient care as appropriate and with all other disciplines within the practice.
  3. To plan and organise own workload, including audit and project work, and training sessions for members of the medicines management team, practice team, community pharmacy team, community nurse team, patients, carers, etc.
  4. To ensure that all patient information following consultations, and telephone encounters are recorded accurately on the computer.
  5. To maintain registration as a pharmacist and comply with appropriate professional codes.
  6. As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process.

About us

The Borchardt Medical Centre is a friendly and busy GP Practice located in Withington, Manchester. We have approximately 10,200 registered patients. All the staff are friendly and we have a supportive working environment where everyone is approachable.

We are using EMIS Web and DOCMAN as our clinical system along with Accurx to provide digital platform to our patients

There is staff car parking available on site.

  • Conduct structured medication reviews (SMRs) for patients with complex polypharmacy, frailty, or long-term conditions.

  • Manage and monitor long-term conditions (e.g., hypertension, asthma, diabetes) under appropriate clinical supervision.

  • Support safe deprescribing and medicines reconciliation following discharge or clinic visits.

    Essential

    • Registered Pharmacist with the General Pharmaceutical Council, GPhC.
    • Evidence of continuing professional development CPD.
    • Minimum of 2 years post-qualification experience
    • In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare
    • An appreciation of the nature of primary care and the diversity that exists between general practices
    • An appreciation of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
    • Can plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions

    Desirable
